Aztecs believed those who succumb to illness or old age went to the Aztec underworld, Mictlan. However the Sultan cursed the protagonist with cancer meaning, upon death, he is confronted by Tezcatlipoca about his malignancies. The Sultan, who resembles the Grim Reaper, promised the wanderer that he would descend into an ancient Aztec kingdom one of the thirteen Aztec Heavens. Meanwhile drummer, Brann Dailor, acts as a narrative vocalist throughout the song.
